Once the lesson starts students should hopefully be engaged and ready to start the lesson.

Perhaps the class is large and some students are less inclined to speak up when they need help, or some are reluctant to participat.
For students who are struggling, try pairing them with students who show greater strength with the material, and it will help both partie.
Pairing and grouping students will also help students in a larger class get to know each other and feel more comfortable participatin.
Be sure that English is spoken as often as possible, or at least when it is appropriate in accordance to the class or students' leve.
Teachers should always remind the students to use English if they respond to an activity in their native tongue, and should only respond to what they say in Englis.
Obviously a student speaking their native language is not out of insubordination but just a desire to communicate, which should be kept in min.
